Plantar Fasciitis: What to Do When Stretching Is Not Enough

Plantar fasciitis usually causes heel pain with the first steps in the morning or after sitting. Pain may improve as the foot warms up and then worsen after prolonged standing or activity.


Why stretching may not be enough


Calf and plantar fascia stretching can help, but chronic symptoms often reflect a broader load problem. Footwear, training volume, calf strength, body weight, work demands and foot mechanics can all contribute.


Strengthening and shockwave


Progressive calf and foot strengthening can improve the tissue’s ability to tolerate load. Shockwave therapy has evidence for chronic plantar fasciitis and does not require an injection. Ultrasound can confirm thickening and rule out other causes.


Other options


Selected patients may benefit from image-guided procedures, PRP or other interventions. Steroid injections can reduce pain but require caution because repeated or poorly placed injections may increase the risk of plantar fascia injury.


When to seek care


Persistent pain, progressive weakness, recurrent swelling, locking, inability to bear weight after an injury, or symptoms that limit sleep and daily activity deserve medical evaluation. Treatment is most effective when it is based on a clear diagnosis rather than simply the location of pain.
